On completing the membership form please ensure you attach two passport-style photos.

One photograph will be kept for our records. The other will be used on your official membership card.

Please note: If you do not supply TWO photographs we are unable to send out your membership card. Without your card you cannot take full advantage of the discount offers.

DATA POLICY: Personal details provided by you in connection with your application will be held by Middlesbrough Football Club in accordance with the Data Protection Act 1998. The details will be held and used so that your application can be processed and your membership administered during the forthcoming/current season. We would like to send you details by post of Middlesbrough Football Club products, up to date information on what is happening at the Boro together with occasional special promotions. To do this we will use your personal details and may disclose them to contractors used to undertake our own products and services including mail sorting services. We will not disclose them to any third party who provide other products or services that are not Middlesbrough Football Club products or services. If you do not wish to receive any of the above information then please notify us on your application form.

Terms and Conditions

• Once the application form has been received by MFC and the payment accepted the member and/ or the member's parent or guardian will not be entitled to any refund if they wish to cancel their membership.
• Membership of the Boro Junior Lions 09/10 will expire on May 31 2010.
• Application for membership of the Boro Junior Lions 09/10 must be made before March 1 2010. Any applications received after that shall be not accepted and the applicant will be sent an application form for the Boro Junior Lions 10/11 when the same is available.
• Mascots are chosen at random from the Junior Lions database and must be aged between six to 12 years.
• All Competition winners will be notified in writing using the contact details you have supplied.
• For free entry into reserve home games all members under the age of 14 must be accompanied by an adult.

NO ONE UNDER THE AGE OF 14 WILL BE ALLOWED ENTRY UNACCOMPANIED
